Highly modular 25-liter medical backpack designed for military first responders, mobile medical teams, and configurable trauma-care loadouts.
The Zentauron ZFR — Zentauron First Responder — is a modular medical backpack designed to scale from a relatively lightweight first-responder setup to a much more comprehensive field medical loadout.
According to Zentauron, the ZFR was developed with support from military training personnel in Hammelburg, with the goal of creating a medical pack suitable both for first responders within a unit and for mobile medical teams.
The result is a platform built around modularity rather than a fixed internal layout. The entire interior is lined with loop fabric, allowing compatible dividers, medical organizers, and pouches to be positioned according to the mission and individual medical concept.
Made in Germany.
25-Liter Medical Pack
The ZFR provides approximately:
25 L / 1,526 cu in
of internal capacity.
This gives the pack enough room for a substantial medical loadout while remaining considerably more compact than large expedition-style medical packs.
Depending on configuration, it can be used for:
- First responder equipment
- Trauma supplies
- Dressings and bandages
- Airway equipment
- Diagnostic equipment
- IV / access supplies
- Medical consumables
- Personal protective equipment
- Other mission-specific medical gear
Medical supplies are not included.
Fully Modular Interior
The entire internal compartment is lined with loop fabric.
This allows compatible hook-backed accessories to be positioned throughout the interior, including:
- Medical organizer pouches
- Internal dividers
- Elastic equipment holders
- Transparent medical modules
- Specialized equipment inserts
- Other hook-backed organizers
The layout can therefore be rebuilt according to the mission rather than forcing the user into a permanently sewn internal configuration.
Adaptable From Light to Heavy Medical Loadouts
One of the main concepts behind the ZFR is scalability.
The backpack can be configured as a:
- Lightweight first responder pack
- Tactical medical backpack
- Expanded trauma pack
- Mobile medical-team pack
- Mission-specific medical platform
Users can add or remove internal modules and external accessories depending on the required mobility and medical capability.
Large Front Multislot Compartment
A large hook-and-loop-secured slip compartment is integrated into the front of the backpack.
This separate storage area can be used for equipment such as:
- Ampoule organizers
- Medical documents
- Flat equipment
- Checklists
- Patient documentation
- Other quick-access supplies
This provides additional storage without consuming space inside the main medical compartment.
External MOLLE / PALS Expansion
MOLLE/PALS attachment loops are positioned on the:
- Sides
- Bottom
- Lid
This allows additional equipment to be added externally without occupying the primary medical compartment.
Compatible accessories can include:
- Utility pouches
- Medical accessory pouches
- Side pouches
- Tourniquet holders
- Intervention pouches
- Other MOLLE/PALS equipment
Dedicated Tourniquet Attachment Positions
Additional modular attachment points are positioned toward the lower 5 and 7 o'clock areas of the pack.
These can be used to position tourniquets or other immediately accessible equipment externally.
This allows critical items to remain accessible without opening the primary backpack compartment.
Modular Lid
The lid includes:
- Removable hook-and-loop handle
- Hook-and-loop identification area
- MOLLE/PALS attachment loops
This provides options for identification and additional external equipment.
Patches can be used to identify:
- Medical role
- Unit
- Contents
- Personnel
- Other operational markings
Padded Carry System
The ZFR uses a padded backpack harness designed for carrying heavier medical equipment.
The carry system also includes an interface for the optional ZFR Smartbelt, available in padded and unpadded versions.
Adding a compatible Smartbelt can help transfer more of the load toward the hips instead of carrying the entire pack weight on the shoulders.
Smartbelt sold separately.
Expandable ZFR System
The backpack can be expanded with additional Zentauron components.
Optional accessories include:
- ZFR Smartbelt – Padded
- ZFR Smartbelt – Unpadded
- ZFR Intervention Pouch
- Universal Backpack Side Pouch
- Compatible internal organizers and dividers
This allows the ZFR to be built around different medical roles rather than requiring the same configuration for every user.

Medical Equipment Notice
The ZFR is a medical equipment carrying and organization platform.
The backpack itself does not provide medical treatment capability. Its function depends on the medical equipment, supplies, organization modules, and appropriately trained personnel used with it.
Mission Frontline Take
The ZFR is interesting because Zentauron did not design it as a backpack with one permanently defined medical layout.
The fully loop-lined interior means the same 25-liter shell can be configured for very different roles — from a relatively light first-responder setup to a substantially more comprehensive trauma loadout.
That is especially useful for organizations where different medics do not necessarily carry identical equipment.
We also like the ability to expand the pack externally rather than immediately moving to a larger backpack. The Smartbelt, Intervention Pouch, and side-pouch options allow capacity and weight distribution to be adjusted as the loadout grows.
For users looking for a German-made modular medical backpack that can be built around the mission rather than a fixed factory layout, the ZFR is one of the more serious systems in the Zentauron range.
